Lipid Metabolism and a Sympathico-adrenomedullary System in Patients with Ischemic Heart Disease

In order to investigate the role played by the sympathico-adreno-medullary system in pathogenesis of atherosclerosis, serum lipid concentrations such as free fatty acids (FFA), including their movement under various conditions, triglycerides (TG) and total cholesterol (TC), were determined in patients with ischemic heart disease (IHD). The results are as follows.<BR>The values for FFA (P<0.05), TG (P<0.01) and TC (P<0.01) obtained in a fasting state were significantly higher in patients with IHD than in control subjects.<BR>An intravenous infusion of noradrenaline (NA: 0.15γ/kg/min for 30 min) or adrenaline (A: 0.1γ/kg/min) was followed by a marked elevation of serum FFA, but there was no difference in the elevation between the patients and the control subjects.<BR>Master's double exercise test resulted in a significantly greater rise in FFA (P<0.05) in the IHD group, and a similar tendency was observed following an intravenous infusion of tyramine (20γ/kg/min for 15 min). Irrespective of the patients and controls, the increase in FFA after Master's double exercise test was positively correlated to basal TG concentrations (P<0.05).<BR>Within one hour after an intravenous injection of glucose (0.25g/kg), a decrease of FFA in the patients remained in normal range or a somewhat greater decrease was observed in some of them.<BR>A significantly (P<0.01) negative correlation was observed between FFA increment after NA and FFA decrement after glucose administration.<BR>It was suggested by these results that the cause of abnormalities in lipid metabolism, such as hyperlipidemia frequently seen in patients with atherosclerosis, was attributed to some extent to the function of the sympathico-adrenomedullary system, i. e. a hyperreactivity of lipolysis to endogenous cathecholamines. Furthermore, a relationship between FFA and glucose metabolsim was discussed.
