An Enzymatic Activity of Alcohol Dehydrogenase in the Serum of Patients in Critical or Intensive Care Units

Alcohol dehydrogenase (AD, EC 1. 1. 1. 1) and leucyl aminopeptidase (LAP, cytosol aminopeptidase, EC 3. 4. 11. 1), which exists in hepatocyte's cytoplasma as the rather specific enzymes, have been reported as a good index for hepatic centrilobular injury (Clin Chem, 30: 1817-1820, 1984). Using the sera from 55 inpatients in critical or intensive care units, the enzymatic activities of AD, LAP, transaminases such as AST and ALT, and lactate dehydrogenase (LD) were measured. The mean values of the activities were evaluated in sera from those inpatients such as acute myocardial infarction, each for survived and died after cardiopulmonary arrest, and miscellaneous groups. Most of inpatients showed increased in AD activities. The highest value in AD was observed for the patient group died by cardiopulmonary arrest, whereas the highest activities of LAP, AST, ALT and LD for the miscellaneous patient group which showed higher value than 100 IU/I in AD. From these results, an activity of AD appears to be more useful than that of LAP as an index for the detection of hypoxic hepatic injury at early stage of symptom.
