Interferon was administered in 3 cases of chronic hepatitis B. Nucleoside analogues were not recommended in all 3 cases because of age under 35, and/or their hope of obtaining children based on the Japanese Guideline for the treatment of chronic hepatitis B. Every 3 case had familial clustering of HBV and possessed HBV genotype C. All 3 cases were hospitalized and given 6 million unit of natural interferon alpha for 28 days as an initial therapy. Case No. 1 was 30-year-old married male and showed the seroconversion of HBeAg followed by normalization of transaminases after twice a week and then once a week of the same interferon. Case No. 2 was 33-year-old married female, in whom liver biopsy showed CH (F4/A2), and was given twice, then once a week of the same interferon followed by 90 μg of weekly peginterferon alpha2a and showed normalization of transaminases without HBeAg seroconversion. Case No. 3 was 40-year-old single male, in whom liver biopsy showed CH (F3/A3). He was given natural interferon alpha twice a week followed by 90 μg and then 180 μg of weekly peginterferon alpha2a that resulted in both unexpected biochemical and virological breakthrough. The superimposition of oral entecavir exhibited stabilization of transaminases in this case. IL28B genotyping revealed major homozygote in the former two and minor homozygote in the latter, strongly suggesting the presence of a significant predicting host factor in viral and biochemical breakthrough in interferon therapy.
