THE PRESENT STATE OF MASS SCREENING FOR GASTRIC CANCER USING ENDOSCOPY

In Japan screening for gastric cancer using X-rays has continued for more than 50 years but the maintenance has become less certain since the use of photofluorography has decreased during clinical examinations. It has been suggested that endoscopy should be substituted as the method for gastric screening but such a shift has had some difficulties. The percentage of endoscopy for mass screening failed to reach 5%. The first reason for the low percentage was that the mortality reduction from gastric cancer was not evaluated for endoscopic screening. The trials proving mortality reduction have begun little by little. Now we might recommend endoscopic screening to local governments with a higher detection rate, higher predictive value, higher rate of early gastric cancer detection and lower cost in identifying one case of gastric cancer compared with X-ray screening. Endoscopic screening must be performed under precise management. The introduction of nasal endoscopy has appeared favorable for reducing endoscopy-related pain. A study to evaluate the risk for gastric cancer by blood testing is one of the ways to settle the argument.
