EXPERIMENTAL STUDY ON AN ARTIFICIAL TRACHEA MADE FROM COLLAGEN-GRAFTED MESH.

A new type of tracheal prosthesis made from mesh was studied. It consists of fine Marlex mesh®(pore size, about 300μ, polypropylene) and teflon yarn, which is inserted as supporting rings, and it is coated and grafted with pig collagen. Complete surgical resection and replacement of a 4-6 tracheal ring segment of the cervical trachea was performed in 13 adult mongrel dogs. Except for two dogs which suffered complications due to anastomotic insufficiency, the prostheses in all dogs were infiltrated by the surrounding connective tissue and the anastomotis sites. Formation of respiratory epithelium, which lined the prosthesis lumen, was seen to various degrees, and in one dog which was sacrificed at 4 months after reconstruction, this was confirmed histlogically from the oral to the anal side of the prosthesis. However, stenosis of the lumen often occurred due to deformation of the prosthesis and overgrowth of granulation tissue, and furthermore, ulcer formation and airleakage were seen occasionally. We conclude that it is necessary to improve the processing of the prosthesis, especially the technique used for insertion of stents and the collagen-grafting method.
