<B>Purpose and Methods:</B> The purpose of this study was to investigate the morphological changes by pulmonary vein isolation (PVI) in AF patients. Method: 50 consecutive patients (mean LAD 35.5 mm, mean LA volume 117.3 cm3) undergoing PVI for AF were enrolled. We measured following parameters before and after PVI. The vertical distance from the level of fossa ovalis (FO) to the orifices of PVs (LS-v=LSPV, LI-v=LIPV, RS-v=RSPV, RI-v=RIPV). The horizontal distance from the level of FO to the orifices of PVs (LS-v=LSPV, LI-h=LIPV, RS-h=RSPV, RI-h=RIPV). We set up a reference point in the center of FO defined the intersection point of horizontal plane and sagital plane by MDCT. <B>Results:</B> LS-v and RS-v were decreased significantly after PVI (LS-v; from 42.4±5.5 mm to 36.9±5.1 mm, RS-v; from 30.5±5.1 mm to 26.5±6.8 mm). LS-h and LI-h were decreased significantly after PVI (LS-h; from 29.2±4.6 mm to 25.6±5.1 mm, LI-h; from 32.2±4.7 mm to 28.6±6.6 mm). LI-v, RI-v, RS-h and RI-h were not changed significantly. <B>Conclusion:</B> These results suggested that the partial reverse remodeling by PVI was observed at the left superior antrum.
