Para-Hisian Atrial Tachycardia with Fibrillatory Conduction Unidirectional to Left Atrium during Catheter Ablation

A 66-year-old man underwent electrophysiological study and catheter ablation for symptomatic atrial tachycardia and atrial fibrillation. He had been initially treated with verapamil and Class IC. He was referred for catheter ablation due to persistent symptoms despite medical therapy. No evidence of underlying cardiac disease was noted on echocardiography and multislice coronary CT. His ECG during tachycardia exhibited negative P wave in leads II, III and aVF with a duration of 300 ms cycle length. During EP study, atrial tachycardia was induced by program stimulation and changed into atrial fibrillation. Atrial regular tachycardia in right atrium still sustained at para-hisian site with fibrillatory conduction unidirectional to left atrium. Catheter ablation at para-hisian site terminated atrial tachycardia driving fibrillatory conduction to left atrium. We report this unidirectional atrial fibrillatory conduction and the mechanism will be discussed.
