Heart failure (HF) is a trending up pathology in aging populations of Taiwan and Asia. HF is manifested poor quality of life and early mortality as well as increase burden to health care system. Analysis from National Health Insurance Database in Taiwan, there are 0.5 million patients suffering from HF. Monitor in worsening of HF is an important issue to reduce hospitalization and mortality. There were two aspects of HF monitoring: self-monitor and device monitor. Patient education on self-monitor included medication compliance, symptom monitoring, dietary control, fluid and alcohol restriction, weight management, exercise, smoking cessation, preventive behaviors, et al. However, patient age and education level have impacts on patient compliance. We organized a Quality Control Circle team to educate HF patients and got satisfactory results, such as reduce HF hospitalization and health care costs. In this lecture, we would like to share our experience in Taichung VGH. Device therapies (CRT/ICD) have emerged as adjuncts to optimal medical treatment for people with HF. Device monitor of HF has an important role in alerting patients to seek for medical advice by evaluation of fluid status by impedance changes. Device monitoring could provide physicians and nurses with important information, which assist in patient management. In conclusions, self and device monitor of HF would improve management of HF, thus improve patient quality of life and reduce health care burden.
