Background: ICD implantation can prevent sudden cardiac death (SCD) in patients with Brugada syndrome. Nonetheless, patient selection with regards to primary prevention in this group of individuals is often challenging. Objective: We aim to evaluate the incidence of VF as a surrogate endpoint for SCD in our cohort of Brugada patients. We also sought to evaluate the mean duration to the first appropriate shock since implantation. Lastly, we would like to know the incidence of inappropriate shocks in this cohort. Method: A retrospective analysis from the registry of 29 ICD implantations for patients with Brugada syndrome over the past decade in IJN. We had them categorized into 3 groups ie (a) asymptomatic patients (n=6), (b) patients with history of syncope (n=11), and (c) patients with documented VT/VF or survivors of resuscitated SCD (n=12). Results: During the mean follow up period of 53 months (min: 4mths, max: 161mths), none of the asymptomatic patients developed cardiac event, 1 patient in group (b) (9%) developed VF, 10 patients in group (c) (83.3%) had VF. 1 patient in asymptomatic group had inappropriate shocks. Conclusion: Though this study is limited with a small number of patients, they were followed up with a good duration of time. It clearly demonstrated that the risk of SCD is low in asymptomatic patients with Brugada syndrome.
