Iatrogenic Pericardial Effusion and Tamponade Late after Pacemaker Implantation

Cardiac tamponade as a late complication of cardiac device implantation was rare, but serious complication. A 78-year-old man was underwent pacemaker operation with atrial and ventricular screw-in pacing leads by transvenous approach for management of sick sinus syndrome. On the first postoperative day, atrial fibrillation was occurred, and then he began to take warfarin. 7 days after operation, he complained chest discomfort and blood pressure was decreased. Echocardiogram showed pericardial fluid. Pericardiocentesis was not performed, because hemodynamics became stable immediately and pericardial fluid was not increased. Furthermore lead position did not change in chest X-ray. He was discharged from our hospital 10 days after operation. The 49 days after discharged, he was readmitted to our hospital because of hemodynamic instability. Pericardial fluid was increased and echocardiogram showed cardiac tamponade. Pericardiocentesis was performed and 750 mL of bloody fluid was aspirated. Since then, pericardial fluid no longer increased for 5 months follow up. <B>Conclusion:</B> These findings suggested that delayed tamponade was complicated by minor leak into the pericardium via screw of lead and warfarin administration. When screw-in pacing leads were implanted, warfarin administration immediately after operation should be cared.
