We report the case of a 66 year old man with dilated cardiomyopathy who received a cardiac resynchronization therapy with a defibrillator (CRT-D: Concerto, Medtronic, Minneapolis US) in October, 2008 for sustained ventricular tachycardia (VT) and left ventricular dysfunction. After implanting, he had two times CRT-D discharge in 2008. After that, there is no episode of discharge of CRT-D. The pacing ratio was 94.7% and pacing amplitude was 2 V/0.4 msec in RA, RV and LV leads. In August 2010, routine device interrogation shows the battery reach recommended replace time (RRT). In September 2010, device exchange was performed and we request an investigation this device to Medtronic. As a result of the analysis of this device, it was finding of the crack in the power supply filter capacitor. This report is the first description of the crack of the capacitor of CRT-D device. This abnormality might be able to be forecast by carefully observing the capacity of the battery.
