Characteristics of Patients with Asymptomatic Atrial Tachyarrhythmia Detected as Atrial High Rate Episodes by Pacemaker

Asymptomatic atrial tachyarrhythmia (AAT) including atrial fibrillation and flutter is considered to be related to the occurrence of stroke or cardiac death. AAT is detectable as atrial high rate episodes (AHREs) in patients with pacemaker. To clarify the characteristics of the patients with AAT, we assessed the factors predisposing to AAT. <B>Methods:</B> We studied 213 patients (100 males, age 76±10 years) with pacemaker implanted for sick node dysfunction (SND) or atrioventricular block (AVB) but without any symptomatic atrial tachyarrhythmia. Clinical and echocardiographic variables immediately after pacemaker implantation were analyzed in light of the presence of AAT after pacemaker implantation. Pacemaker recognizes AHREs when the atrial rate reaches >180 bpm and AAT was defined as AHREs for >1 minute or longer. <B>Results:</B> During the follow-up periods (12±6 months), AAT was recoreded in 41 pts (19%). AAT was recorded in 28 pts (25%) with SND and in 13 pts (13%) with AVB (p<0.05). There was no difference in age, gender, echocardiographic variables, history of stroke, or incidence of hypertension, diabetes mellitus, ischemic heart disease and valvular heart disease between those with and without AAT. <B>Conclusions:</B> Asymptomatic atrial tachyarrhythmia occurrs more frequently in patients with sick node dysfunction than in those with atrioventricular block. Factors predisposing patients to asymptomatic atrial tachyarrhythmia may be different from those to symptomatic atrial fibrillation.
