<B>Background:</B> These days we can see some reports about knot impingement after rotator cuff repair. The complaints of patients almost all disappear by removal of the knots. But because their complaints are very light, there are patients who do not want removal of the knots by arthroscope. We report the natural course of patients with knot impingement after rotator cuff repair.<BR><B>Methods:</B> We performed arthroscopic rotator cuff repair (ARCR) from May 2006 to April 2011. Among the patients who underwent surgery and passed over 2 years after getting diagnosis as knot impingement, 9 patients did not want removal of the knots on the rotator cuff. Knot impingement was diagnosed by T2 MRI. The evaluation of the patients was done by clinical complaints and physical examination, plain X-ray, and MRI. The average age of the patients was 68.4 years old (52-78), two men and seven women.<BR><B>Results:</B> Almost all the complaints of patients, for example dullness and crepitation, have not changed during the evaluation period. Effusion in subacromial bursa has not disappeared in MRI of any of the patients. In some patients, the under surface of the acromion has been curetted over time. But in some other patients, the under surface of the acromion has been sclerosed.<BR><B>Discussion:</B> If knot impingement occurs, the knots should be removed. But in some cases, the under surface of the acromion has been sclerosed after getting diagnosis of knot impingement. It may be possible that we do not have to remove the knots in such cases.
