• <B>Background:</B> The purpose of this study was to compare the clinical results of the arthroscopic patch graft procedure and arthroscopic partial repair procedure for irreparable massive rotator cuff tears in patients who had low-grade fatty degeneration of the infraspinatus (stage 0 to 2, according to the Goutallier scale).<BR><B>Methods:</B> 14 shoulders (8 males, 6 females) underwent the arthroscopic patch graft procedure using an iliotibial band or a tensor fascia lata (group PG) and 12 shoulders (5 males, 6 females) that underwent the arthroscopic partial repair (group PR). The clinical outcome was evaluated using JOA score. Muscle strength was measured with MicroFET. Postoperative rotator cuff repair integrity was determined through MRI and evaluated using the classification of Sugaya.<BR><B>Results:</B> The average JOA score improved to 88.5 points in Group PG and to 84.5 points in Group PR (p<0.01) postoperatively. Muscle strength significantly improved in both groups. No statistical differences were found between the two groups in regard to postoperative scores and muscle strength. Postoperative MRI revealed 7 type I, 4 with type II, 1 type IV, and 2 type V in Group PG, and 2 with type III, 4 type IV, and 6 type V in Group PR.<BR><B>Conclusion:</B> For the treatment of irreparable massive rotator cuff tears with low-grade fatty degeneration (stage 0 to 2) of the infraspinatus, both procedures yielded successful outcomes. However, the arthroscopic patch graft procedure was found to be superior to the arthroscopic partial repair procedure in terms of postoperative repair integrity.
