The mental states of four epileptic patients not receiving medication for long periods of time (10 years on the average) were examined using the Rorschach test.
Two of the four cases suffer from primary generalized epilepsy and the other two from partial epilepsy. One of the partial epilepsy cases showed a temporal spike focus. Impaired consciousness attacks increased gradually in the partial eplepsies, while generalized tonic clonic convulsions decreased after the age of 25 in the primary generalized epilepsies.
Responses to the Rorschach test differed from normal responses in all four cases. There were fewer responses or response time was longer or form levels were distorted. One case suffering from PGE showed emotional instability and extroversion, characteristics of PGE previously reported by the author. A case suffering from TLE showed previously reported TLE characteristics of inefficiency and a constrictive tendency.
The present paper sugests that the character of PGE and TLE are not influenced by antiepileptic therapy. When anticonvulsants were prescribed and the patients became completely free from seizures, the character of TLE was seen to be much improved while PGE showed less change.
The intelligence of the two PGE patients was average as seen in Rorschach scores. One of these patients who has experienced 50-100 generalized tonic clonic convulsionshad a WAIS IQ of 98. These results support Gastaut's view that IQ does not fall in PGE.
