Surgical treatment of the basilar artery aneurysm has been one of the difficult surgery even if surgery is performed under the operating microscope.<BR>The authors reported that the surgical approach to the basilar artery aneurysm had to be discussed, especially in the case of a high placed basilar artery aneurysm. There are four approaching routes to a basilar artery aneurysm by pterional approach, namely routes 1 and 2 is through the space between the internal carotid artery, and the optic nerve or the temporal lobe. Route 3 is through the triangular space between the internal carotid artery, the optic chiasma and the A<SUB>1</SUB> portion. Route 4 is through the triangular space between the A<SUB>1</SUB> portion, M<SUB>1</SUB> portion and the base of the frontal lobe. The authors reported that the routes 3 and 4 are the easier approach to the high placed basilar artery aneurysm.
