Clinical investigation on surgical treatment of severe hypertensive intracerebral hemorrhage (lateral type) have been carried out in 10 patients. Preoperative state of consciousness was stupor or semicoma. Four patients (40%) died within 19 days after operation. Three of these four patients were semicomatous pre-operatively. One (14%) of seven patients who were stuporous preoperatively also died. In stuporous group (7 patients), there was a close correlation between postoperative results and preoperative neurological findings such as abnormality of pupils, light reflex and oculocephalic reflex. One patient with negative oculocephalic reflex died. Six patients with positive oculocephalic reflex survived. In this group, four patients with anisocoric pupils and negative light reflex became vegetable, however two patients with isocoric pupils and positive light reflex became alert.<BR>It is still premature to make a conclusion but we recommend to operate on patients having any voluntary movements preoperatively (stupor), because patient in this state of consciousness still has chance of recovery after operation
