In order to clarify the relation between stress and hemoptysis, the author measured the urinary excretion of 17-OHCS in pulmonary tuberculosis 35 patients, who had the tendency to hemoptysis (the "far advanced" category and the more serious of the "moderately advanced" of the A. N. T. A. classification), and compared the values with their normal values in the quiescent period.
While the average daily excretion of 17-OHCS was found to be 4.2 ± 0.3 mg in normal subjects, it was 2.3 ± 0.4 mg in these patients. The figure for those of the patients who actually showed hemoptysis was 2.2 ± 0.3 mg in the quiescent period and the average of the highest values observed during the period of hemoptysis was 3.7 ± 0.6 mg. The excretion in the three days immediately preceding the onset of hemoptysis could be observed in nine patients, of whom one was observed twice. These values were distinctly higher, in all the cases except two than the average of three measurements made during the quiescent period, which was 2.1 ± 0.4 mg. The average of the highest pre-hemoptysis values was 3.9 ± 1.2 mg.
The differences between the figures are statistically significant excepting the difference between the values before and during hemoptysis.
The fact that these patients have higher urinary 17-OHCS values a few days before the onset of hemoptysis seems to indicate that some unidentified factor appears in the body and acts as a stress, causing increased excretion of 17-OHCS on the one hand and the onset of hemoptysis on the other.
