We are studying on the endocrinological circumstances of this disease systematically, in order to investigate the entity of so-called Metropathia haemorrhagica. Author made his studies of this disease, from the standpoint of adrenocortical function by systemic estimation of the urinary 17-OH and 17-KS, in these patients, and compared with the result those obtained from the non-pregnant healthy women of corresponding ages. Then, the histological examination of the adrenal cortex of albino rats with persistent estrus, which were believed to be endocrinologically in the similar circumstances as Metropathia haemorrhagica in human beings, was performed, and the result was compared with those examined before the beginning of bleeding. Furthermore, the fluctuation of urinary 17-OH and 17-KS excretion of these patients following administration of gonadotropin or sex steroids, were investigated.<BR>As to the urinary 17-01-1 excretion of the non-pregnant healthy women, the difference due to age and menstrual cycle was not seen, while the urinary 17-KS excretion, which differed from age, was highest in 23 to 39 years old, and increased at the time of ovulation.<BR>Observing the fluctuation of urinary excretion of these two hormones in the patient before bleeding, the 17-OH value was generally low and temporarily increased at the beginning of bleeding, while the 17-KS value was comparatively high more or less a week before the bleeding and decreased gradually down to the beginning of it. During the bleeding, both 17-OH and 17-KS were distinctly low, especially by the women of 21 to 39 years old., <BR>Comparing these results above mentioned with those of the author's co-workers, who estimated the urinary gonadotropin and estrogen excretion in same patients as the author used, it was found that the comparatively high 17-KS value before bleeding and the increase of 17-OH at the beginning of it, were preceded by peaks of gonadotropin and estrogen respectively. The fact, that the 17-OH value was low contrastedly, when the 17-KS value was high before bleeding, seemed to show the phenomenon of so-called "Sekretionsumstellung" by Tonutti.<BR>The histological examination of adrenal cortex of albino rats in persistent estrus, showed the hyperplasia and hypertrophy of cells in juxtamedullary zone, and hyperplasia of cells in fascicular zone, fine and depletion of lipid granules. These findings indicated the hyperfunction of these two cortical zones. The former finding coincided with the result, that the 17-KS value was high before bleeding, while the latter contradicted the low value of 17-OH before bleeding.<BR>Our trial to induce the ovulation in the patient of this case by the administration of gonadotropin by our original method, succeeded in 4 cases out of 5, and the increase of 17-KS was seen at the period corresponding to ovulation by the successful cases, while no such change was seen by the unsuccessful case. Progesterone caused temporary increase of both urinary 17-OH and 17-KS of these patients, certain days after the administration, while estrogen caused increase of 17-OH only during or after the administration. Furthermore, the author experienced remarkable increase of the 17-KS but decrease of the 17-OH by androgen administration.<BR>Comparing these experimental data above mentioned with those of co-workers, who made studies on the anterior pituitary and ovarian functions employing the same materials as used in this report, the author reached to a conjecture that the adrenal cortex may be also concerning with the anterior pituitary-ovarian dysfunction in Metropathia haemorrhagica.
