The therapy with progesterone is considered to be the most reasonable treatment of functional bleeding, but we could not obtain good results with usual preparations of low dosage inspite of the neccessary repetition of injections. Of late, Pots and Boschann have successfully treated this condition by a combination therapy of estrogene and progeesterone in high dosage according to the Kaufmann's theory.
In our examination, we administered 125mg l7-alpha-hydroxyprogesterone capronate and 10mg estradiol benzoate simultaneously and could attain a significant success. We carried out this therapy in 8 ambulant patients of functional bleeding who came to our gynecological clinic of the Osaka University Hospital. Success was achieved in 7 patients whose bleeding completely stopped within 48 hours. The remaining one showed a remarkable decrease in amount of the bleeding. An after-bleeding appeared about on the 6th day of the treatment and lasted for 4-7 days, but not any other undesirable side-effect was observed.
We made the examinations of basal body temperature, cervical mucus and vaginal smear after the injection along with a clinical observation of the course of the condition, and we found that the following menstrual cycles became regular in the cases, in which an ovulation was confirmed. A recurrence of the condition was not observed during the 2 to 3 months' observation. There was also no change in the amount of pregnanediol excretion in the urine due to the injected hormones. We recognized a secretory transformation of the endometrium after 7 days, which had showed a picture of proliferation phase before the injection. From the results of this examination, it may be considered, that the after-bleeding experienced in all the cases is probably a withdrawal bleeding due to the injected hormones.
