It has been talked much about that stress promotes the genesis of the peptic ulcer since Selye's general adaptation syndrome. It is confirmed experimentally that psychological troubles have great influences upon the genesis of the ulcer. These psychological reactions by external stimulus are frequently influenced by the personality of receivers. The condition of autonomic nervous reaction has also an influence indirectly upon the genesis of the ulcer.
From these points of view, the psychosomatical consideration is necessary in regard to the genesis of the peptic ulcer. And also, in postoperative complaints, psychosomatical factors are considered as the subject as compared with organic factors. And even if there are cases where organic factors are considered as the subject, it may be considered that psychosomatical symptoms appear on grounds of organic symptoms.
The auther tried to investigate the patients' mentality, personality and autonomic nervous system psychosomatically on the genesis of the peptic ulcer and postoperative complaints. The patients with benign gastric lesions who were admitted to the Showa University Hospital for the purpose of gastrectomy were divided three groups ; preoperative, postoperative and pre- and postoperative group, and were examined by means of MMPI and mecholyl test.
The results were as follows :
1) Preoperative group : The patients with peptic ulcer showed more tendencies to neu-rosis and vegetative instability than those with other gastric lesions, and had much psychosomatical factors.
2) Postoperative group : Neurotic tendencies were recognized in 80 % of male and 100 % of female of dumpers, and turned to patients with postoperative complaints and those with non-complaint. In mecholyl test, P types were seen in 65.4 % of dumpers and in 52.9 % of patients with postoperative complaints. So, postoperative complaints were closely related with vegetative instability.
3) Pre- and postoperative group : Postoperative neurotic tendencies were larger than preoperative neurotic tendencies in postoperative complaints group, but smaller in Dumping syndrome group and non-complaint group. And, preoperative neurotic tendencies had great influence upon occurence of postoperative complaints, and particularly, in Dumping syndrome.
