A 61-year-old male presented with the chief complaints of pain of the left cheek. CT scan revealed a mass in the left sphenoidal sinus. The sphenoidal sinus was treated with irradiation but other tumors developed in the pelvis and liver, and the patient died. The initial pathological diagnosis of biopsy tissue taken from the sphenoidal sinus was squamous cell carcinoma, but the autopsy diagnosis was hepatocellular carcinoma. The pertinent literature on metastasis of hepatocellular carcinoma to the head and neck region is reviewed.
