The morphological changes in the vestibular ganglion and crista ampullaris of guinea pigs were investigated after intratympanic treatment with gentamicin sulfate 4mg (0.1ml) for 5 days.One day after treatment, the crista ampullaris showed decreased numbers of sensory hairs in the central portion, and the degeneration gradually spread peripherally. Two weeks after treatment, the sensory cells had almost disappeared, while the nerve fibers showed no morphological changes.Three days after treatment, the satellite cells of the vestibular ganglion cells showed vacuolization, and the myelin sheaths around the ganglion cells were loosened, split and interrupted.One week after treatment, the cisternae of the granular endoplasmic reticulum were dilated in the peripheral portion of the ganglion perikaria. This change became more marked and was observed throughout the cytoplasm three weeks after treatment.Regressive processes in the nerve fibers were not observed throughout the examination period, suggesting that the early degeneration of the ganglion cells after treatment was due to the direct action of gentamicin.
