Vestibular neuritis with multiple attacks of vertigo.

There is some disagreement as to the diagnostic criteria for vestibular neuritis. In Japan, vestibular neuritis is defined as a disorder of the vestibular system with a single attack of vertigo. Some foreign authors include cases of multiple attacks of vertigo.We studied the clinical and neuro-otological findings of three patients with multiple attacks of vertigo. A 36-year-old man (Case 1) experienced four episodes of vertigo during a two-week period. The first three attacks each lasted about 10 hours, and the last attack subsided gradually over a period of two weeks. Examination revealed spontaneous nystagmus to the left and greatly decreased caloric response in the right ear. Audiometric tests showed normal hearing.A 40-year-old man (Case 2) experienced severe vertigo of sudden onset which persisted for six hours. One week later he had a recurrence of severe vertigo and was confined to bed for four days. Spontaneous nystagmus to the left was observed. Caloric tests showed decreased response in the right ear. Hearing was normal.A 34-year-old woman (Case 3) had a sudden attack of vertigo which lasted for one hour. Eight hours later she had another severe attack which lasted about 16 days. Examination one month after the attack revealed direction-changing rotatory positional nystagmus, nystagmus to the right during eye closure, decreased caloric response in the left ear and normal hearing.The clinical findings in these three patients conform to the Japanese diagnostic criteria for vestibular neuritis except that attacks of vertigo were multiple. The histological findings of the vestibular nerves of patients with a single episode of vertigo are similar to those of patients with multiple episodes of vertigo. We conclude that vestibular neuritis may have multiple episodes of vertigo for a limited period of time and need not be limited to cases of a single attack of vertigo.
