A 47-year-old female who sustained head injury developed symptoms suggestive of endolymphatic hydrops as well as benign paroxysmal vertigo. She showed high and low tone deafness in the left ear with a positive sign of the glycerol test. Administration of Isosorbide (90ml per day for 5 weeks) showed a beneficial effect in relieving cochlear symptoms, particularly loss of hearing. Positional nystagmus persisted after above treatment, although it changed from horizontal to rotational in type. In parallel with this, vertigo gradually decreased. From these findings, we suspected that both endolymphatic hydrops and benign paroxysmal positional vertigo were possibly brought about by the head and neck injury. However, further investigation is necessary to determine as to whether endolymphatic hydrops so induced can be an etiological factor in eliciting benign paroxysmal positional vertigo.
