A case, which ended fatally with brain symptoms and the protrusion of the right eyeball after operative treatment of the mucocele of the frontal sinus and the obliterated ethmoid sinuitis, was thoroughly studied from both the clinical and the histo-pathological view points and the following conclusion was come to:<BR>The cancer started in the mucous menbraae of the anterior part of the right ethmoid cells, invaded the orbit gradually in one direction and the fore- brain through the cranial basis in the other, perforating the less resistent parts of the ethmoid walls i. e. the lamina papiracea and the lamina cribrosa.<BR>It was also evident that the clinically assumed mucocele of the right frontal sinus was only a secondary effect due to the, obliteration of the ductus naso-frontalis and that the tumor was a Krompecher basalcell carcinoma.
