Clinical and Experimental Studies on the Diagnosis of Stomach Cancer using Gastrocamera

There is no radical therapy of stomach cancer except by surgical operations. It is most important to detct stomach cancer in the early stage. The author has studied on the diagnosis of stomach cancer using Gastrocamera in a man and an experimental applicatcion of ultraviolet photography on gastric mucosa in human resected stomachs, dogs, and mice in order to improve the diagnostic ability of gastrocamera.The results are as follows :A. Clinical Studies. One hundred and fifty-nine patients (male 112, female 47) were operated in the Tokyo University Hospital from Jun. 1955 to May. 1960, because of stomach cancer.1) Among them, 121 patients received either palliative or curative resection. 2) The Types of stomach cancer were divided into 4 groups by gastrocamera : ulcerating form (32.0% of the patients who received gastrocamera examinations), cancerous ulcer form (54.1%), tumor form (4.7%), and infiltrating form (10.2%). 3) The ratio of male to female was 5 : 2. The incidence of stomach cancer between 20 to 40 years of age was higher in women. 4) Patients who had ulcerating form tended to have complaints for long periods. 5) Three main symptoms of stomach cancer were epigastralgia, epigastric fullness and the loss of weight. 6) It was relatively easy to diagnose gastric cancer except ulcerating form with gastro camera. There exists many problems to differentiate ulcerating form from benign ulcers. The main diagnostic points were as follows : irregularity and bleeding of the floor of ulcer, vagueness and bleeding of the edge, broad spread of rednness into the surroundings, uneveness, irregularity and concurvature of the ridge, cessation of folds, and deformity of the arch.B. In earhy stomach cancer. 1) 17.3% among the resected cases were found to be stomach cancer in the early stage. 2) Predisposing position was the anterior wall of the body. 3) Most of the patients had complaints, such as epigastric pain, fullness, and distress, for a long time. 4) Characteristic f eaturers of early stomach cancer were i) relatively large and shallow ulcer with whity coats, ii) irregular margin which was invaded by cancer cells, iii) the cessation of folds. 5) Most of them were classified as the ulcerating form 6) Good results were obtained using gastrocamera.C. Experimental studies on the ultraviolet photography of stomach mucosa. 1) In mice ultraviolet photography revealed more precisely details of experimentally produced malignant ulcer by means of the inplantation of Ehrlich's cancer cell into the stomach mucosa than that of experimentally produced benign ulcer by means of the injection of Lantan-gel into stomach mucosa. 2) In the resected stomachs, ultraviolet photography revealed many characteristic featurers of malignant ulcer, 3) In the resected stomach of cancer in early stage, ultra-violet photography had many advantages. Above all, it revealed very clearly the form of ulcer, the extend of invasion of cancer cells, and the cessation of folds.
