A woman, 56 years old, had for about 5 years complained of dull pain in her left knee and noticed a localized persistent swelling in the knee for about 2 months before admission. At examination, soft elastic mass was palpated. A biopsy was done and the tissue was interpreted as PVS.The synovectomy was performed and expanded mass and reddish brown villous tissue was resected. Microscopically, hemosiderin pigment with marked histiocyte and proliferation of lining cells was found, but not foam cell. This case was not typical because of the absence of foam cell but we consider as PVS.
