There are two sesamoids found in the tendons of the flexor hallucis brevis under the first metatarso-phalangeal joint. They articulate with two longitudinal grooves planter to the first metatarsal head and stabilize the metatarso-phalangeal joint during propulsion. Disabling pain may occur under the first metatarsal head following various conditions such as trauma, degeneration or osteonecrosis of the sesamoids. However, aseptic necrosis of the sesamoid is a rare entity. Renander in Acta Radiologica (1924) was the first to report this entity. He reproted two cases and described the histology as osteochondropathy. Roentgenographic findings were fragmentation, mottling and irregularity of the sesamoid. In this report we present two patients with the diagnosis of aseptic necrosis of the sesamoid.
