A new technique for reduction and fixation of severe spondylolisthesis is presented. This consists of skull-femoral traction, specially made body cast and wire traction from the anteriorly displaced vertebral body. In this wire traction, wires are pulled down through the window which is opened in the body cast. After achieving reduction of the displaced vertebral body, anterior interbody fusion is done. Postoperatively wire traction continues for about 6 weeks. All of the 11 cases treated by this method from 1982 to 1984 had satisfactory results. There was no remarkable complication in this series.In conclusion, this technique may be useful in the treatment of severe spondylolisthesis.
