A case of tuberculous infection of the C2 vertebral body with retropharyngeal abscess is described. To avoid the fistula formation into pharynx, the abscess was drained through the anterolateral approach at first, then, debridement of necrotic bone and curettage of the lesion was performed by a transoral approach. Although simultaneous debridement and bone grafting are usually made in such cases, we did not resort to bone grafting because we expected the effect of antituberculous chemotherapy and reliable drainage. But the result was satisfactory.
