The introduction of an arthroscope into the subacromial bursa and other joints (elbow, ankle, wrist, and minor joints) requires a high degree of dexterity and familiarity and becomes more and more difficult with increasing caliber of arthroscope.Since 1974, we have performed arthroscopy of the subacromial bursa and these other joints in almost 200 cases. Inserting an arthroscope into the subacromial bursa, in particular, can only be accomplished by skilled and experienced hands, as a result of our failed attempt in occasional cases. Such circumstances prompted us to develop an instrument for introducing a arthroscope into the subacromial bursa and small joints safely and with certainty, which we designated Fukuoka University's Step Cannulation System (abbreviated to S. C. S.).S. C. S. consists of an 18-gauge needle, guide wire, a series of 7 inner trocars if inner diameters increased and a series of 7 corresponding outer trocars. S. C. S. can also make use of an arthroscope of different caliber, e. g., 2.7mm arthroscope and 2.2mm Watanabe scope.This system was used in arthroscopy of 88 SAB, 12 elbow joints, 8 ankle joints 5 wrist joints, and 1MP joint, and 1 temporomandibular joint proving useful in facilitating insertion of arthroscope in all instances. In cases undergoing arthroscopic operation with the use of this system, it was found advantageous to employ an arthroscope of greater caliber in enhancing manipulability of operative instruments.
