The natural history of idiopathic scoliosis.

A retrospective review of 78 patients with idiopathic scoliosis was performed to determine what factor might be useful in predicting the risk of curve progression. We studied components such as curve patterns, chronologic age, Cobb's angle, Risser sign, and vertebral wedging at diagnosis. A single thoracic curve and thoracolumbar curve had a greater incidence of progression. The risk of curve progression in the Risser 0, I, II, III was approximately 40%-50%, while only 16% of the patients with Risser IV, V progressed. Chronological age and curve magnitude at diagnosis was not related with curve progression, but, there was a clear relation ship between vertebral wedging and curve progression in the skeletally immature curves.Growth potential and vertebral wedging are useful indicators of curve progression.
