Rupture of the Radial Collateral Ligament of the Fifth Metacarpophalangeal Joint: Three Cases Report.

Isolated rupture of the radial collateral ligament of the metacarpophalangeal (MP) joint of the fifth digit is uncommon in comparison with collateral injury of the thumb. We have managed three cases in last one year. Two cases were recent injuries that were treated within 3 weeks after occurrence and one case was longstanding injury. All patients complained of sever pain, swelling and tenderness at the radial side of the fifth MP joint. An ulnarward stress test, with the MP joint in a flexed position, demonstrated lateral instability of the joint. Arthrogram showed leakage of dye on the proximal side of the MP joint in two cases and on the distal side in another case. Complete rupture of the radial collateral ligament and rupture of the dorsal capsule were identified in all cases. We repaired the ruptured collateral ligament by means of end-to-end suture in two recent cases and pull-out wire technique in a longstanding case. We had excellent results in one case and good in two cases.
