Traumatic Inferior Dislocation of the Shoulder due to Tear of Rotator Interval.

We report a case of traumatic inferior dislocation of the shoulder due to tear of rotator interval and discuss therapeutic approaches.The case was a 16-year-old man who while playing volleyball, jumped at the ball and fell. His left arm was forced into abduction and he noted immediate pain and restriction of shoulder movement.On physical examination, tenderness and swelling were observed along the shoulder, and range of motion was severely limited. Roentgenograms showed inferior dislocation of the left shoulder in the resting position and an arthrogram showed dye projection into the rotator interval.Surgery was performed, with exploration of the rotator interval which was widely opened and the anteroinferior glenoid labrum and middle glenohumeral ligament attached. The coracohumeral ligament was not found. Glenoid labrum and middle glenohumeral ligaments were repaired and rotator interval was closed in the position of internal rotation.The post-operative course was satisfactory and limitation of external rotation did not occur, with the patient able to return to sporting activities.
