Tsushima island is located 120km from Kyushu, making it difficult for patients with hip fractures to leave, and there are only three hospitals on the island. We conducted an epidemiological study of hip fractures in Tsushima island, as it is possible to obtain exact data on this condition within such confires. We obtained data both on cases treated surgically or conservatively from hospital records and roentgenograph films. Subjects were 112 hip fractures (27 males and 85 females, 47 cervical fractures and 65 trochanteric fractures) occurring from April, 1988 to March, 1993.Trochanteric fractures were more common than cervical fractures in females aged over 75 years. The incidence of hip fractures per 100000 person-years was 51.3 and 133.9 for males and females 50 years of age and older respectively. This incidence is lower than that reported elsewhere in Japan. In Tshushima, primary industry was dominant with the primary labor ratio of 41.6% (Japanese average ratio was 7.2%).We believe that this difference in work environment may affect the drop in incidence of hip fracutures seen in Tsushima.
