Complication and Prognosis of the Femoral Neck Fractures in Patients Over 90 years old.

Surgical treatment for 20 femoral neck fractures in 19 patients over 90 years old were evaluated. Seventeen patients could walk and 12 patients lived in their own house before fracture. All patients had some complications (Dementia in 10 patients, hypertension in 4 and chronic obstructive pulmonary disease in 3) before surgery. Surgical treatments included 12 osteosynthesis (Ender nailing in 5 cases and captured hip screw in 7) and eight prosthetic replacements. The follow-up period ranged from 3 months to 5 years 1 month averaged 2 years 5 months.Postoperative complications were pneumonia in 6 patients, cardiac failure in 2, infection of prosthesis and acute myocardial infarction in one each. Two cases died within 1 month after surgery. Four patients regained walking ability and 13 could move by wheelchair. One patient remained bedridden. Sixteen patients needed to stay in health care facility. Only 2 patients returned home. Survival of our patient is comparable to the life expectancy of the Japanese in same age. Surgical treatment of the femoral neck fracture for the patients our 90 years old did not shorten their life but improved their quality of life.
