The pancreas in most cases of the portal hypertension is characterized by the hard induration and the tone of red color, also demonstrating glucose intolerance.In this paper, the pancreas was studied histopathologically in 95 surgical biopsy cases afficted with portal hypertension, and a comparison was made between the laboratory findings.Histologically, there were dilatation of capillary, irregularity of elastic fiber in venule, intra- and interlobular fibrosis, hemosiderosis, irregularity in size and shape of islets, hypertrophy of islets, and hyalinization of islets in the pancreas of portal hypertension. It is considered that the chronic pancreatic congestion in portal hypertension is one of important factors involved as the cause of pancreatic fibrosis.The hyalinization of islets were often noted in cases with glucose intolerance. There was highly significant correlation between glucose intolerance and hyalinization of islets.It is considered that the hypertrophy of islets is due to increasing requirement for insulin in liver dysfunction.The marked histological changes were recognized in both exocrine and endocrine glands of the pancreas in portal hypertension. Therefore, it is very important for the longterm management of portal hypertension to take care of pancreatic functions, as well as liver functions, before and after the operation.
