A pharmacokinetic study was performed in 24 cirrhotic patients using cefpiramide (CMP), an antibiotic of the cephem class.<BR>Significant increase of area under the curve and prolongation of half life and decrease of body clearance were noted after intravenous injection of CPM. Consecutive injection of CPM to cirrhotic patients resulted in a higher steady state serum concentration and even higher levels in those with renal dysfunction. As liver dysfunction progresses, urinary recovery of CPM was increased but compensatory urinary excretion was not enough to improve the pharmacokinetic parameters in cirrhotic patients.<BR>These data suggest that there is a substantial impariment of drug metabilsm in liver cirrhosis and provide us a caution in drug administration to these patients.
