Liver tissues taken surgically from 10 patients with PBC were studied for the subsets of the infiltrated lymphocytes and plasma cells by means of the enzyme labeled antibody method using Leu series antibodies and antibodies to immunoglobulins. T cell subsets in the peripheral blood were also measured by immunofluorescence method using antibodies of OKT series. OKT4/OKT8 ratio in the peripheral blood were less than 1.5 in 4 cases of early stage and over 1.5 in 3 cases of late stage of PBC. In both portal and lobular areas, T cells were dominant over B cells. Inportal areas and lobular areas, Leu 2a-positive cells were dominant over Leu 3a-positive cells and in the epithelial cell layer of bile ducts with CNSDC, Leu 2a-positive cells but not Leu 3a-positive cells were found. Among plasma cells, IgM-containing cells were observed most frequently, which infiltrated around the bile duct and also into the epithelial cell layer of bile duct with CNSDC. These results could suggest that cytotoxic T cells might be responsible for the damage of CNSDC and IgM-containing cells might be responsible for the increased serum level of IgM in patients with PBC.
