Pathological studies on the fibrous capsule of so-called encapsulated hepatocellular carcinoma.

The author investigated the structure of tumor capsule of so-called encapsulated hepatocellular carcinoma (HCC) morphologically and microangiographically. Materials studied were 54 cases (39 autopsy and 15 surgical cases) of HCC in all of which fibrous capsule formation around HCC was noticed macroscopically. Histologically, the author classified the tumor capsule into two categories, namely truly encapsulated HCC and pseudo-encapsulated HCC. The former was surrounded by hyalinized fibrous tissue almost completely and the latter had no or only localized hyalinized fibrous tissue around HCC. Capsular structure in the truly encapsulated HCC associated with liver cirrhosis was consisted of three layers, and two layers were determined in cases without liver cirrhosis. The thickness of the inner-most hyalinized fibrous tissue (1st layer) was varied considerably from a portion and a portion even in the same tumor nodule. No remarkable relation between the thickness of the hyalinized fibrous capsule and the size and the histological grade of malignancy of HCC was determined. It was confirmed that the zonal structure of the tumor capsule in truly encapsulated HCC was fairly well Corresponded to the clinical evidence with diagnostic imaging-procedures. From the results obtained, the author wants to propose that the category of the encapsulated HCC should be limited to the truly encapsulated HCC.
