A case of cystadenoma of the liver is reported, stress being laid the usefullness of immunohistochemical investigations for CEA, CA19-9 and Vimentin to define the lesion. The histological examination revealed three distinct layers of the cyst wall: (1) an epithelial layer of mucin producing columnar or cuboidal cells lining the cyst lumen; (2) a layer of undifferentiated meenchymal cells beneath the epithelial lining; and (3) a dense layer of collagenous connective tissue just outside the stromal band of cells, which met the original criteria of Edmondson, et al. Eight cases of cystadenoma of the liver from the Japanese literature were reviewed, but only four cases including ours, all female, had features compatible with the original criteria. Immunohistochemically, CEA and CA19-9 were mainly demonstrated on the apical membrane of the epitherial cells, and Vimentin was positive in the densely cellular stroma. Although mucinous cystadenoma and/or cystadenocarcinoma of the ovary or endometrial cyst show histological resemblance to cystadenoma and/or cystadenocarcinoma of the liver, CA19-9 was negative among those extrahepatic lesions, and Vimentin was negative in the endometrial stroma.
