The authors describe a case which suggested that partial nodular transformation (PNT) of the liver is identical to idiopathic portal hypertension. A 55-year-old female patient developed splenomegaly and portal hypertension, and died of rupture of esophageal varices. PNT was diagnosed based on the presence of coarsely nodular hyperplastic changes at the perihilar regions of the liver. The histopathology of the liver was similar to that of idiopathic portal hypertension in the following findings: subcapsular parenchymal atrophy, circular fibrous enlargement of the portal tract, collapse and narrowing of the portal vein, and systemic portal sclerosis of the middle and large portal vein. Histometrical study of the liver also showed morphological similarities between the two conditions. The pathogenesis of nodular changes was unknown, however, the collapse and narrowing of the peripheral portal vein was observed both in the subcapsular and perihilar regions, while the degree of the patency of the hepatic artery was almost same as that of the normal controls. It was suggested that compensatory hyperplasia of the hepatocytes for the subcapsular atrophy might focally develop in the hillar regions to form nodular changes. Maintained hepatic arterial flow in the hillar regions might contribute to the genesis of the parenchymal hyperplasia.
