Serum hyaluronic acid and type IV collagen levels were evaluated in various liver diseases, and were compared with the pathological diagnosis by liver biopsy for their usefulness in the differential diagnosis of chronic hepatitis (CH) and liver cirrhosis (LC). In addition, we evaluated these usefulness in predicting the therapeutic effect of interferon-α (IFN) in the treatment of CH type C. At the level of 130ng/ml for the cut off value of hyaluronic acid, the diagnostic efficiency was 89.5%, and the specificity was 90.1%. When its cut off value was evaluated in combination with cut off value of type IV collagen (250ng/ml), its diagnostic efficiency as well as screening capabilities were enhanced. Patients who had a pretreatment serum hyaluronic acid level of above 100ng/ml were not effective to IFN therapy. From these results, it is suggested that serum hyaluronic acid and type IV collagen levels could be useful in differentiating CH from LC, and that the measurement of serum hyaluronic acid level before treatment may be useful in predicting the efficacy of IFN therapy.
