In order to clarify the multicentric origin of hepatocellular carcinoma (HCC), nodular lesions less than 20mm in diameter found in 51 autopsy cases of liver cirrhosis (LC) and in noncancerous areas of 60 resected small HCCs associated with LC were pathomorphologically studied. According to the morphologic definition of muleticentric occurrence of HCC by The General Rules for the Clinical and Pathological Study of Primary Liver Cancer, the incidence of synchronous multicentric HCCs in liver cirrhosis was 4 (50%) in autopsy cases, and 9 (15%) in surgical cases. If adenomatous hyperplasia (AH) can be regarded as precancerous lesion of HCC, the multicentric occurence was thought to be in 6 (75%) among the autopsy cases and 13 (22%) among the surgical cases. This study suggested that many small HCCs detected during the follow-up study of cirrhotic patients could be multicentric occurrence.
