肝疾患における血液凝固能に関する実験的・臨床的研究

Hepaplastintest (HPT), thrombotest (TT), prothrombin time (PT) and blood coagulation factors II, V, VII, VIII, IX and X were measured in patients with liver disease, in mice with MHV-2 hepatitis and in dogs with Eck fistula. The following results were obtained.<BR>1) The activities of the blood coagulation, except for factor VIII, showed a decrease in liver diseases as compared to healthy subjects, especially in fulminant hepatitis and in liver cirrhosis with coma.<BR>2) In liver diseases, there was a correlation between the decrease of the activities of blood coagulation, except for factor VIII, and the severity of the disease.<BR>3) The PIVKA-inhibitor showed a higher level in fulminant hepatitis than in hepatitis and in liver cirrhosis.<BR>4) In mice with MHV-2 hepatitis, the activities of the blood coagulation decreased with the progression of hepatic necrosis. This tendency was particularly noted in factor VIII.<BR>5) In Eck fistula dogs without meat intoxication, the changes of the blood coagulation factors were within the normal range. But in Eck fistula dogs with meat intoxication or with CCl4 hepatic damage, the activities of the blood coagulation, except for factor VIII, greatly decreased.<BR>Therefore, it is believed that the determination of blood coagulation tests in liver diseases is useful to determine the diagnosis and prognosis of the diseases, and that involvement of DIC was suggested in fulminant hepatitis from the aspect of PIVKA-inhibitor and factor VIII.
