肝膿瘍を合併した大腸癌の1例

We experienced a case of liver abscess accompanied by sigmoid colon cancer. A 69-year-old male was admitted to our hospital because of fever up and right upper abdominal pain. A diagnosis of liver abscess was obtained from the results of abdominal ultrasonography and CT as well as clinical findings. Antibiotic therapy achieved an improvement in the symptoms. After that, colonoscopy and barium enema were carried out in order to determine the source of fecal occult blood. Endoscopic examinmation revealed a tumor with ulceration suggesting colon cancer at 20 cm from anal verge. It was diagnosed as adenocarcinoma from biopsy specimen. High anterior resection and surgical drainage of liver abscess were performed. Histological findings of liver demonstrated periportal infiltration of lymphocytes with no remarkable changes around bile duct. The mechanism how colon cancer secondarily forms liver abscess is still unknown at this time. Infection through portal vein is the most probable. Colon cancer may be taken into consideration when we encounter liver abscess.
