A 42 year old woman was referred to our clinic for anal lesion with pain and pus discharge of two month duration. Patient had a chronic anal fissure with fistula. In addition, a mass lesion in the presacral space was found by digital examination on the left posterior line of rectum. Puncture was performed which revealed a caseous material ; bacterial culture was negative. Tumor was removed by parasacral approach. Multiple cysts sticked together just like a three-leaf clover ; three cysts were all the same size of walnut. Each chamber had different contents, i. e. serous, yellowish pasty and browny pus like material respectively. Pathological examination revealed a multilocular structure covered with stratified ciliated columnar, transitional or stratified squamous epithelium, similar to those of the anorectal junction and anal duct. And a few smooth muscle fibers conformed to the periphery of the cysts in a concentric fashion. All these pathological findings were not contradictory to those of "Tailgut Cysts".
