Pathogenesis of gall stone formation in patients with inflammatory bowel disease.

Bile acids, lipids and nucleation time of the bile in patients with inflammatory bewel disease were measured to investigate the pathogenesis of gall stone formation in these patients. Bile acid concentration was decreased in patients with ulcerative colitis and Crohn's disease who had the ileocecal lesion. Approximately in the half of the patients, lithogenic index was above 1.0, but it did not correlate with gall stone formation. Nucleation time was shortened in all the patients except ulcerative colitis treated conservatively and closely related with gall stone formation. The mechanism of shortened nuclation time was thought to be mainly to decreased intestinal absorptive function. Studies of facilitating or inhibiting factors of nucleation time, such as approtein A- I, A- II, mucin or calciumion will be necessary to confirm further conclusion.
