The topic of rectal prolapse will be discussed in detail. Current theories regarding etiology as well as a proposed classification system will be given. The role of anorectal physiology studies in establishing the diagnosis of "hidden rectal prolapse" and in assessing the sphincter mechanism is also discussed. The, various transabdominal and perineal operations for procidentia will be reviewed. Although similar recurrence rates have been reported for many of these techniques, the functional results in terms of incontinence or constipation may be quite different. The importance of accurately assessing continence before and after prolapse operations is stressed. The University of Minnesota experience with over 200 cases of massive prolapse will be discussed. For the good risk patient an intraabdominal proctopexy without foreign material has resulted in a low recurrence rate and good sphincter function. The poor risk patient (average age 79 years) has been successfully managed with a perineal rectosigmoidectomy.
